Sports Physical Information

Our school sports include: Football, Volleyball, Flag Football, Basketball, Baseball, and Softball. We also have Cheerleading, Dancing Paws, and Majorettes. Come out and support our mighty Bobcats!

Baker County Middle School has gone to an online platform for submitting the FHSAA Physical Packet that began with spring sports 2022. If your student has a current physical, you still need to go online to the athletic clearance website and create a free account (if you do not have one). If you need to submit a new or updated physical or packet, please go to www.athleticclearance.com or click HERE, click on FL, and follow the directions (CLICK HERE FOR A VIDEO TUTORIAL) on how to create your FREE ACCOUNT. All the forms you need are located on the site and can be printed from the site as well as below. If you need to print just the physical form, you may find it below. All students MUST have insurance in order to participate. If you need insurance information, you may find it below. An email address is required to submit the paperwork for athletic clearance.

Once you have submitted the physical, please DO NOT SEND A HARD COPY to BCMS, keep it for your records. BCMS will have access to it through Athleticclearance.com
